Services Will Be Provided for 6 Months From The Date of Release

Things Boston Release Network Would Like Prior To Release Date

1.       Inmate’s Name

2.       Inmate’s date of birth

3.       Inmate’s release date

4.       Copy of psych/social (This document is a necessary part of the application to many programs and shelters)

5.       Copy of CORI (This document is a necessary part of the application to many programs and shelters)       

6.       If applicable, a copy of inmate’s DD-214 (early receipt of this document will help us secure a bed and services at Vet’s shelters)

7.       Inmate’s MassHealth number

8.       Information regarding inmate’s physical health and any special needs i.e., wheelchair, no stairs, etc., and a list of their medications (this information will help us narrow down housing and services that would be available or helpful)

9.   A list of upcoming medical/dental/mental health appointments (this will allow us to schedule transportation)

10.   The name and contact number for persons or agencies that are guaranteeing services, or shelter (this will allow us to confirm the inmates release plans)

Things That The Inmate Needs To Receive Upon Release

1.       Birth Certificate

2.       Social Security Card

3.       Activated MassHealth Card (with mental health component if applicable)

4.       Massachusetts ID

5.       DD-214 (if applicable)

6.       30-day supply of medication (including insulin, needles, test strips, and glucometer if applicable)

7.       A list of upcoming medical/dental/mental health appointments that have been scheduled by HSU

These are the services that we can provide to inmates being released from your facility. We can:

On the day of release:

  1. Pick up the person from the facility and transport him/her to Boston;
    1. Contact the proper Registry Officer and set up an appointment for the person to register;
    2. Transport the person to the proper Probation Department to be fitted with a GPS unit if it is Court ordered;
  2. Coordinate with your property officer to set an acceptable time and date to pick up the person’s property;
  3. Assist with setting up a bank account, if necessary;
  4. Facilitate entry to the shelter;
  5. Introduce him/her to their support team;

The following days:

  1. Help them activate their MassHealth if it hasn’t already been done;
  2. Confirm medical appointments;
  3. Help them apply for Food Stamps;
  4. Help them apply for additional benefits they may be eligible for;
  5. During the first few days develop a realistic release plan with the person in order to better facilitate their reintegration to the community.
